8/6/2023 EnchantaVaughn Approved Behavior: When the QMHP-C entered the household, Laylah personality was viewed as cheerful, polite, respectful, and obedient. Laylah displayed helpful and supportive behaviors towards her mother as she assisted her inside of the home. Laylah communicated pleasant and kindhearted manners during the session. Laylah was in a much better mood today than her previous moods. Laylah verbalized positive feelings. Laylah displayed good listening skills. Laylah was very attentive to the instructions and guidelines of the activity. Laylah demonstrated well-mannered behaviors. Behavior: Laylah implemented the rules and expectations while processing and reflecting on her day in a one on one conversation with QMHP-C. Laylah negative behaviors were stable and calm, at this time. Laylah was able to manage her emotions correctly. Laylah stated she felt good and was prepared and couldn’t wait for her baby brother to arrive. Laylah understood her responsibilities as a big sister and enjoyed helping mom around the house. Laylah exhibited positive behaviors instead of the negative behaviors she often exhibits when she does not receive the attention she desires. Laylah was able to reduce symptoms of ADHD by staying calm. Intervention: Before starting the session, the QMHP-C complemented Laylah on her well-mannered and mature attitude that she had displayed throughout the session. QMHP-C utilized a combination of mood regulation techniques, to help Laylah delay the need for her immediate gratification and inhibit her impulses. QMHP-C provided active listening skills. QMHP-C identified the positive consequences Laylah has experienced in response to delaying empathy and treating others respectfully. QMHP-C examined and evaluated Laylah’s demeanor in order to manage her compliance with following rules. Intervention: QMHP-C continued to work with Laylah to help her increase respect for others. QMHP-C asked Laylah to identify, role-play, and model appropriate social skills during session. QMHP-C provided positive words of praise and reassurance to encourage and promote confidence and respect for herself. QMHP-C asked Laylah open ended questions to get her to interact and increase participation. QMHP-C reminded and practice breathing and relaxation techniques to teach Laylah the importance of the “stop, think before act” method. QMHP-C continued to support Laylah and meet her needs. Response: Laylah was receptive and open to the information that was provided by QMHP-C, as she listened attentively to all that was presented to her regarding her behaviors. Laylah’s mom was approachable and open-minded to the feedback given by QMHP-C. Laylah was able to express her feelings and moods in a positive way without getting upset. Laylah stayed on task and returned a sociable and friendly rapport back to mental health counselor without any negativity. Laylah followed the instructions and remained focus and active during the therapeutic activity. Laylah was cooperative. Response: Laylah fully participated in role-reversal techniques during the session. Laylah was able to demonstrate positive interaction and excellent manners without having an episode. Laylah said thank you. Laylah displayed high self-esteem and confidence. Laylah appropriately answered QMHP-C open-ended questions. Laylah was observed practicing breathing and relaxation strategies and mastering the “stop, think, act” method. Laylah was acceptive of QMHP-C support. Laylah followed all directions, as she demonstrated ability to adhere to the rules and expectations of individual session. QMHP-C Plan of Action: QMHP-C will continue to assist Laylah in respecting the home, interacting and engaging appropriately with her peers and relatives, and continue to review her behavioral plan and goals daily. QMHP-C will implement active listening, following directions, problem solving, and coping skills. QMHP-C will assist Laylah with maintaining a calm demeanor in stressful situations through modeling positive behavior and psycho education.
